Current Cigarette & Tobacco Retail Licenses will expire on June 30, 2024. With the recent passage of Wisconsin Act 73, there have been changes to the forms related to cigarette and tobacco licensing. Please review the below information regarding forms:

• Form CTV-100 is now the new and renewal license application.
• In addition to the main application, form CTV-101 is required for sole-proprietors; all officers, directors, and agent of a corporation; all partners of a partnership; all managing members and agent of a limited liability company.
• If the applicant is an LLC or corporation, form CTV-102 must also be submitted.
• Proof that the applicant holds a seller’s permit. This can be a copy of the seller’s permit document. You can submit a digital or physical copy of your account information on the DOR website under My Tax Account, then Search Account Number under the Business section. 

Please fill out the required forms and return them to the City Clerk's Office in addition to the $25 license fee by May 17, 2024 to ensure that gaps in licensing do not occur. License applications received after this date may take longer to process and issue. Payments can be made via cash, check, or credit card with a 3 percent service fee.
