Alcohol Beverage Retail Licenses

Current Alcohol Beverage Retail Licenses will expire on June 30, 2024. With the recent passage of Wisconsin Act 73, there have been changes to the forms related to alcohol beverage retail licensing. Please review the below information regarding forms:

 

  • Form AB-200f is now the new and renewal license application.
  • In addition to Form AB-200f, include:
    • Form AB-100, Alcohol Beverage Individual Questionnaire, for all individuals listed in part C
    • Form AB-101, Alcohol Beverage Appointment of Agent, for corporation, nonprofit organizations, and LLC applicants
    • Responsible beverage server training course completion certificate or other acceptable replacement document described in Part B, Question 5
    • Proof the applicant holds a seller’s permit, such as a copy of the seller’s permit document. You may also submit a digital or physical copy of your account information on the DOR website under My Tax Account, then Search Account Number under the Business section.

 

Please fill out the required forms and return them to the City Clerk's Office in addition to the appropriate license fees by May 17, 2024 to ensure that gaps in licensing do not occur. License applications received after this date may take longer to process and issue. Payments can be made via cash, check, or credit card with a 3 percent service fee. Please contact the City Clerk's Office at 920-648-2344 with questions or concerns.
