Applications can be submitted to the City Clerk's Office at 200D Water St Lake Mills, WI 53351 via mail, drop box, or in person on or before May 17, 2024 to ensure gaps in licensing do not occur. License applications received after this date may take longer to process and issue. Payments can be made via cash, check, or credit card with a 3 percent service fee. Applications will not be reviewed or approved without the submittal of a certificate of insurance in the amount of $1,000,000.00, naming the City of Lake Mills as an additional insured
Attach a scale drawing that shows the specific seating to be used and the method to be utilized to separate the dining and serving area from the remainder of the public sidewalk. Dimensions must be shown. Please note that an unobstructed width of 60 inches must be available to the general public on the sidewalk at all times
